* Please fill out this form, then click the submit button. You will then be able to pay for your registration. Look for the payment link on the next screen. Your registration is NOT complete without payment. You will receive an automated email confirming your payment. This is also your ENTRY CONFIRMATION for the ORT. Entry method is first received.

Please Note:

  • Final details will be emailed about 5-10 days before the ORT. Emails will be sent using a email service called YAMM, so please white list shop@thek9nose.com with your email provider, and watch to see that the confirmation email is not in your SPAM or Promotions folder if you use gmail.

  • The ORT Title must be earned at least 14 days before a trial opening date to be eligible for the first draw period. All odors must be passed in order to earn the ORT title. It is not required that they all be passed at the same test.

  • Please contact your host at least 1 day before the ORT if your female dog will be in season.

  • All confirmations will be sent via an automated e-mail confirming your payment. This is also your ENTRY CONFIRMATION for the ORT. Entry method is first received. If you require confirmation via USPS, you must provide a self-addressed stamped envelope.

  • By filling out this form and clicking on the “submit” button, I/We hereby assume all risks of, and responsibility for, accidents and/or damage to myself or to my property or to others, resulting from the actions of my dog. I/We expressly agree that Yas Bakshian, The K9 Nose, Fusion Pet Retreat, 14901 Minnetonka Industrial Road, Minnetonka, MN, 55345 and/or NACSW or any other person, or persons, of said groups, shall not be held liable personally, or collectively, under any circumstances, for injury, and/or damage to my person, for loss or injury to my property, whether due to uncontrolled dogs or negligence of any member of said groups, or any other cause, or causes.
